Drug Formulary

Who decides which drugs are in the formulary?

Drugs considered for Arise Health Plan’s Drug Formulary are evaluated by the Pharmacy and Therapeutics Committee (P&T Committee), which includes local physicians and pharmacists, and may include other health care professionals such as specialists, nurse practitioners, and/or physician assistants.

The P&T Committee bases it's decisions on clinical evidence from literature or reference materials, expert opinions, and cost/benefit studies. Information may also be obtained from peer-reviewed journals, pharmaceutical companies, and the FDA.

Important decisions are not left up to national companies. They stay local and are based on quality, effectiveness, safety, and cost to members. Most importantly, the P&T Committee allows for regional nuances in health care, making decisions that are relevant to members in Wisconsin.
