Disease Management
Disease Management is a multidisciplinary, continuum-based approach to health care delivery that identifies people with, or at risk for, chronic medical conditions. Disease management supports the doctor-patient relationship and plan of care, emphasizes the prevention of exacerbation and complications using cost-effective, evidence-based practice guidelines and patient self-management.
- StayWell Health Management
Members with chronic illnesses can receive support for daily living using programs that help delay complications related to the condition. Our disease management program is administered by StayWell Health Management, who provides ongoing coaching, health education, and guidance to help follow a physician’s treatment plans. If you have seen a practitioner for one of the disease conditions, you will receive a letter and call to determine if you are interested in participating in the program. - Enroll in our Disease Management Program
Arise Health Plan offers a disease management program (administered by StayWell Health Management) for members with diabetes, coronary artery disease or heart failure. Program participants may have a health coach and/or receive educational materials. For more information, please fill out this form or call 920-490-6956.
- Prevent Diabetes Problems
Because diabetes can cause eye problems, members with diabetes are encouraged to see their eye care specialist annually for a dilated eye exam. Download this Dilated Retinal Form, ask your eye doctor to complete it, and then bring it to your primary care provider.

- Cholesterol
Medline Plus provides an interactive multimedia tutorial with information on managing your cholesterol, including causes, treatment options, diets, medications and a quiz to test your knowledge. It is published by the Patient Education Institute and licensed by the National Library of Medicine.
- High Blood Pressure
High blood pressure, or hypertension, is a disease typically with no symptoms. The American Heart Association is a national voluntary health agency that helps people learn how to keep blood pressure in the healthy range, reducing disability and death from cardiovascular diseases and stroke.
Use the American Heart Association’s High Blood Pressure Risk Calculator to determine if you are at risk for heart attack, stroke, heart failure, or kidney disease.
Medline Plus provides an interactive tool regarding hypertension, its causes, complications, how it is diagnosed, and ways to control it. It is published by the Patient Education Institute and licensed by the National Library of Medicine.
- Living Well Workshops
Adult members with ongoing health conditions are invited to attend a 6-week Living Well Workshop. Designed to help you feel better and more control, you’ll get the support you need, find ways to deal with pain, discover nutrition and exercise options, and learn how to evaluate new treatment choices.
If you suffer with diabetes, arthritis, high blood pressure, heart disease, chronic pain, or anxiety, you owe it to your health to attend the 2.5-hour workshop, held once a week for six weeks.
